forgot username and password/can't get test results
I have forgotten my username and password. I have been unable to access my health information for a year now. My doctor e-mails my health information such as test results through your website. I can't get the information.

If you have forgotten your user name, you can retrieve that through our Forgot User Name page. You will need your enrollee and group number to complete this form.
